Lake Stockton Healthcare Facility has received a five-star rating from the Centers for Medicare & Medicaid Services (CMS), the highest score available in the agency’s Five-Star Quality Rating System.
The facility, which is owned and operated by Citizens Memorial Health Care Foundation in Bolivar, provides skilled nursing, memory care, and rehabilitation services. The CMS rating system evaluates nursing homes across the country based on health inspections, staffing levels, and quality measures. A five-star rating means that a facility is considered “much above average” in overall quality by CMS.
